I was really the Executive again to me to think that tomorrow starts the peer room which was a celebration of Esther when she with the help of god stop this the plight of Jews being completely annihilates a Tomar starts Piramal we celebrate Esther and today I feel like I'm here for just such a time as this currently Arkansas has the third highest infant mortality rate in the nation and is concealing been going up we also have on the highest rates of maternal mortality rates in the nation as well separate represented Ferguson I passed legislation last year to do some commission studies to find out why why is that happening in Arkansas what we can do to prevent it and make things better in Arkansas well the first thing that I looked at we saw the dentist or what is happening in the states that have the lowest rate of infant and mortality rates so states like Massachusetts Rhode Island Vermont those areas up there they have already passed legislation like this which creates the ability for certified nurse midwives to practice to the full scope of what they've been taught and licensed to do and so those states that did much better than us have already passed this. Days like a she's me Mississippi Louisiana behind us have not I really want to pass this bill and leave states like Mississippi and Louisiana and the dust to make Arkansas better. I know that some of you need to have low education and what it means for midwife when you hear the word midwife you think a maybe a licensed midwife that practices at home by a licensed midwife has only get an education a high school education gone through a program and certified to be a lay midwife certified nurse midwives have got their education as a registered nurse and they go on for this that to get their masters their doctorate so much more educated and ready to provide perform deliveries like they do around the world and stay in the country it's like you're in England in those places nurse midwife deliver eighty five percent of the babies there and then return all death race an infant rates are much better than ours are here Arkansas has the most restrictive a collaborative agreements in the nation that's and that's why we don't have any certified nurse midwives working here we have a sensually close the gate and say you're not welcome to practice here we have women that are leaving the state and driving to Oklahoma and other states that they can have a certified nurse midwife deliver their baby in a birthing center we have certified nurse midwives that live in Arkansas and travel to another state so they can have their job and worked there for years and come back this is a bill that will allow Arkansas to address the things that we know there's things that are happening in our state that are causing the reason that we have infant death rates that we do babies that are born too early babies they have low birth rates those things are creating a higher rate of infant death rates in Arkansas and states that have adopted bills like this allowed certified nurse midwives to practice I have seen a great decrease in those numbers I have seen babies healthier moms healthier and the rates continue to rise so we have some questions about happen to take those on this bill this time. Represent Dalby for purpose question you're recognized I apologize Representative that I didn't ask you this the other day when we were this thing about this what about hospital privileges you have a a mother who goes into into labor has a certified midwives complications occur. But I am not this is way out my real mafia sure it you know what about can they get him to the hospital is those kinds of things have is that work could you explain that to me consent us. Sure in states where certified in midwives practice eighty five percent are living in the hospital they're part of the the medical team the working with collaborating physicians that are already there in the and they already have transfer agreements were if if they do a birthing center which I hope will have some birthing centers because mom's never leaving to have babies there so they have transfer agreements already in place for those cases I had some of my colleagues I was gonna say that to the end of my presentation but I'll read it now we have some colleagues that had some concerns in the committee and asked me to do an amendment I'm going to present this amendment that would reach you on the house and we want to move the move the bill full but I read the amendment about transfer for that so the amendment that I've asked me to and not a problem it's not safe for delivery outside of an accredited facility the certified nurse midwives solidify a licensed physician or facility or both with which an arrangement has been made for referral and consultations the event of a medical complications so she's gonna have those arrangements in place beforehand as part of the nurse practice act have been put in there by this amendment and I agreed it's always been filed and ready to go in the Senate in. There's anybody else that has any questions. Hodges I know some of you get an email with some physicians being concerned that certified nurse midwives were not trying to do is to see your inspections or deliver a baby with four steps and that's true they are not trained for that and they don't want to do that there's the nurse practice act for certified nurse midwives they said they would deliver babies that are low risk in states where they have already passed this bill twenty nine other states they work hand in hand with the positions under there and like I said eighty five to ninety percent of deliveries or the hospital server complication arises they have a physician right there to work hand in hand with to work with that baby on but they're not gonna do sustained sections they're not going to do a delivery with forceps and I will go over with you I mean I've had a baby it was I was blessed to be able to have a certified nurse midwife deliver my first two children and a certified nurse midwives stays with you the whole time it's not like she's going in and out of with the physician under says take care of the the mom goes in their system situations arise and they go to the doctor consult to whatever and maybe if the the labor slows down and the contractions stop a little bit the physician order some protection which increases labor contractions and different thanks as everything is done medically based and doing that with a certified nurse midwife stays there the whole time she's directly with that woman doing things like purely a massage attorneys to she's doing things like changing positions of the make the baby come as a totally different thing physicians do medical a midwife does the art of major for you like to have done for generations for thousands of years their with the women work with them one on one it's much more intimate thing and I know that I'm with the things we found that with our maternal mortality Review Committee is that women of color older women are really one of the highest risks for maternal mortality so would I really release work release with you a message today was I'm encourage. Jing them to sponsor and recruit women of color to become certified nurse midwives in our state because that's one thing when you have that one on one that we can reach out and they can communicate much better I've seen in other states a real decrease in that group well when they've had certified nurse midwives B. them on this deliver and another thing certified nurse midwives have a hundred percent rate of women are breast feeding because after the delivery they're making a twenty four hour visit at their home going to their home a consulting with them see if they're having issues that make a forty eight hour visit and they have a one week visit of two week it's much more intimate and much much closer thing And so the breast feeding rates are much higher than they are with physicians and I in Arkansas if we look and see if anybody knows me for very long because I'm so passionate about increasing the health of the state we have guts to do so much more than we are and this is a little step in the right direction I'm not trying to tell you that this is going to and things in office and tomorrow the infant mortality rates to be better they're not this is a definite step in right direction contextual get we need to what we have got to get our state healthier we're forty eighth in the nation right comes to obesity and so many things and it is scientifically proven if we get children to breast feed that they're they're better healthier for their entire life their moms are better and as women of color have a much but a decrease in hypertension those other things if they're breast feed their babies for a longer time so it to me this is a a win win situation for entire state and also will we know the certified nurse midwives we know that in our state nearly seventy five percent of the deliveries are paid from either Medicaid or the Medicaid expansion population a certified nurse midwife is only going to get paid eighty percent of the positions going to pay there's much less a Syrian burst so economically the state will save some money as well as I think in an option and some better care for our women in our infants and if there's I could talk all day it's a passion offices subject that I'm passionate about this any other questions I'll be happy to take those. My wife and I have four children first one was born in fort Collins Colorado at boulder valley hospital when I was a student there at C. issue. The next two were born at home with the mid one. In our mid life. First name was Cheryl she did such a good job delivering to my two middle children that I was just sold on. My one encouraging me to support her decision to have a midwife present. Later when we got overseas and medical care was unavailable and one of the country's we lived in. We found that we were expecting number four. And there wasn't anyone there that we could call on and so she said Brandt you were present for the middle to hear my midwife and so. We delivered the fourth child at home. And the that girl is a twenty eight years old today and I'm sold on midwives I do want to see good training good support for them and I think this is a good bill a Representative Bentley and I'm fully supportive and I ask you for a good vote. But I am for this and I'll tell you why I'm for it is merry said we both did a fellowship together on infant and maternal mortality in I'm not for home births I mean I think the infants are seven times more likely to die in a home birth and I think there a lot of problems with that but I think the general public really doesn't understand the disparity in education between a certified masters are doctors nurses midwives and allied midlife there's a huge difference so if if people are going to cheese that I want to make sure that they have the most qualified person to do that and is represented Bentley said my main concern I want to make sure that we had a transfer agreement in place for those situations so if you know I mean they are bad things that can happen in normal pregnancies at delivery so I want to make sure that there's an arrangement with the hospital to make that transfer to an obstetrician of but I think it has the potential I mean there's no reason that certified nurse midwives can't deliver normal vaginal deliveries they work in hospitals now in in integrated relationship with base and I think this has the potential to really increase Beecher the state. This bill got labeled a scope bill and it technically was by its original form but when amended died in committee I took out those two paragraphs so I don't really consider being disco bill anymore. What it does is it removes the barrier for physician after write a referral for patients to see a nurse practitioner so the doctors out of town today and you go to the doctor nurse practitioners there they would have to write a referral if you are on Medicaid for you to be able to be seen there is a line in the beginning of the bill this is they can lead and has been brought to my attention that could mean that they would be physicians in the pace of a patient center medical home so I'm willing to amend that to be sent out on the Senate in I've got Medicaid looking at that at the current moment but with that I'd appreciate a good vote answer any questions. So what this bill would do is give them some type of Negotiation to try to recruit I mean there's only a just the numbers I have it's only forty five hundred forensic pathologists across this country. About half of what's really needed so it's a it's a real recruiting battle to try to get a man to bring a law to bring on the Arkansas especially with the pain that we pay here. And I don't know if this if this bill here will be the answer to their problem or not I really don't. But I can tell you this it's a place to start and that's what we need to do because at least give them the ability to negotiate with someone that and I all they're doing is saying look if you come here you're be only years probation. And at the end of that year we will pay twelve thousand five hundred dollars toward your student loan.
