It just worked out, um, this is another landfill bill and what this does is, um, A host community closest to the landfill is what it. It pertains to and right now when a landfill goes for an updated permit. A host community basically just says yes, we're going to give you our certificate of need, but it needs to be done on a regular basis every 4 years. What's happening now is a host fee or host, excuse me, host community will vote and then we don't hear anything back from it sometimes for years and then all of a sudden they will say no, we don't want this. This basically says we're going to do this every 4 years, a consistent. Agreement that you are the host community and that's pretty much it. What the sad thing is now we have landfills that will drag on for years and Things will go well, and sometimes they don't, but this certificate of need needs to be updated that it has landed people in court. This just again brings everybody to the table, updates it. It does not stop any landfill at all. We put the language in here that says, and if you'll look at the very end, there's a specific line in there that just says it doesn't pertain to any current land, excuse me, it does not stop a landfill. The requirement for an additional supplemental certificate of need from the regional solid Waste Management Board and a supplemental host community vote or minute order if more than 4 years passed, adds layers of bureaucracy that could extend the timeline of obtaining necessary permits. This delay could hinder timely waste management solutions and increase. costs for both the applicants and the state. Moreover, the bill's mandate that a host communities supplemental vote, while important for ensuring local input could lead to additional political complications. It's necessary of the necessity of obtaining resolutions or minute orders from local governmental units may result in prolonged debates and potential requests for additional requirements from the host city. To the entity requesting the permit, furthering delay in the process. The subsurface and engineering analysis that need to be performed for these facilities is time consuming and expensive. Costing millions of dollars, adding an additional layer of political risk at the end of the process could discourage investment in critical waste management infrastructure. The host community has already had opportunities prior to the initial resolution or minute order to negotiate host fees with the permit applicant. Review zoning ordinances, requests large cell development plans from the applicant and participate as a member of the Solid Waste District. In summary, while the bill aims to ensure through evaluation and community involvement, the additional time and potential political hurdles it introduces could impede the development of necessary landfill projects. Ultimately affecting the state's ability to manage waste effectively. Um, an example of this, um, on May 10th, the solid waste district approved a certificate of need for Class 1 and Class 4 landfill. The 4 year mark would have been May 22nd. Excuse me, would have been May 10th, uh, 2022. That landfill received its draft permit from ADEQ on July 28th of that year, just past the 4-year mark. So that additional 2 ish months. Before we receive that draft permit, we would have had to go back to the solid waste district, go through that process for a significant need, then go back to the city to request an additional vote or minute order. So an initial 3 to 4 months or more, possibly depending on what the city asked of the entity. So Well, I understand that it does take a long time and that the representative lunchroom and I both would like to see the process expedited. I don't know that this is the best way to do it. Thank

I'm the president of the Arkansas Academy of PA's, uh, where here representing PAs in Arkansas as Representative Gramlich stated, we are highly educated healthcare professionals, and PA schools on average 28 months of education that results in a master's degree that includes over 2000 hours of clinical education through rotations, um, this bill allows for PAs to delegate simple tasks to medical staff, including medical assistants, emergency. and paramedics, uh, currently these tasks are only delegated by a physician, and HB 1131 passed recently, uh, which allowed nurse practitioners to delegate the same way. Um, again, this is allowing MAs and other medical staff to perform tasks that they are trained to do and capable of performing. This is a common practice across the nation and 48 states that we know of allow PAs to allow to delegate these tasks to MA's, uh, PAs are capable of delegating these tasks to nurses and the staff are capable of performing them. when ordered by a physician. So again, this is just kind of streamlining that workflow, uh, reducing delays in care. We have no known opposition, we've worked with the medical society on this bill, um, as well as Arkansas Society of Medical Assistants and with UAMS who has supported our bill. And Harding

So physician assistants, um, we first have a bachelor's degree and then go on to PA school before being admitted to PA school, you have to have um patient care experience, and so different from nurse practitioners where they're required to be specifically an RN, but we do require all of our applicants to have some form of patient care, so whether that's being a CNA or being a patient care tech or a medical assistant, they have to have demonstrated that they've had direct involvement with patients before starting school. Then through school as I said, it's a 28 month program that involves over 2000 hours on our national average um of clinical education, nurse practitioner school on average has about 11,000 hours of clinical care. Now they have the RN experience ahead of that, but as far as clinical experience as a provider, as a nurse practitioner, the average is about 1000. Um, again, we have the over 2000 hours, as far as in practice, the difference, so in in in in our training, we're trained on a medical model, uh, meaning that this kind of res b ol s uh, the training that, uh, medical school is built on as opposed to nurse practitioner school, which is an advanced nursing model. I am a, uh, the program director and chair of the UAMS physician assistant program, but I'm here representing a rapa, a PA organization as the co-legislative chair for our legislative committee, um, and I wanted to speak just briefly, a lot was already said in context about this MA bill, but just to give a little bit of context, with illustrating through my background. I served in the army for 22 years of those 22 years, I was a medic for about half, and then I was a PA for the last half before I retired. Uh, once I retired, I worked a short stint in primary care in San Antonio, Texas, and then, um, got here to UAMS, uh, in 2014 and been a program director since 2017. In that time, being here in Arkansas, I saw a vast difference in how PAs are treated as well as a number of PAs from other states and even through the military. Um, we've been around since 1965 and in that in that time we've grown as a uh profession, not as fast as nurse practitioners because there's a lot more nurse practitioners and nurses and PAs, but in that time that I've been here in in Arkansas, I, when I first started here, there were about 300 licensed PAs in Arkansas. And that was one of the reasons that really motivated me to want to be here in Arkansas and want to work in the PA education to train PAs to the best of their capabilities within the scope of their practice to go out and do good things for the state and to increase those numbers now if you look at the medical board statistics were close to 700 PAs here in Arkansas out there serving communities and doing their best to make a difference. Some of the limitations that we have been able to overcome through legislation has helped tremendously. The biggest impact that I have seen is recent legislation like Act 303 that allowed PAs to be able to be rendering providers for Medicaid makes a difference in them being hired. There's a big issue that we have seen in our program with students, um, graduating that are in state and then they migrate out of state because Oh OK. Uh, because they, they can't find jobs here. This type of bill being passed for PAs will help to make PAs more hirable and more desirable in the healthcare community and make a big difference, and I also can say, although I'm not representing UAMS here, that they do also support this bill.
